Lost Driver Education Certificate

Students who successfully complete the 30 hour Driver Education classroom phase and the 6 hours of behind-the-wheel lessons will be issued a Driver Education Completion Certificate from their BTW teacher on the student’s last day of behind-the-wheel lessons. This certificate must be presented to the DMV for a student to test for their permit.

If a student has lost this form, they may receive a duplicate by coming to the Jordan Driving School offices to request another one. The Jordan Driving School office is open Monday-Friday from 8:30am - 3:00pm.

The following items will be needed for the office staff to issue the student another certificate:

  • Duplicate Certificate fee of $5.00 (Cash Only)
  • Student’s full name as it is on the birth certificate
  • Student's current address
  • The year student completed driver education with a CMS program
